 Registration 2011

There are 4 simple steps to register all of your players:  1) Complete the FAMILY INFO section below, click on the SUBMIT button.  2) Enter your Player(s), 3) Review Registration Information you submitted, 4) Securely pay by Credit/Debit Card.  That's IT!  Cost is $85 per player which includes a team shirt.

SPONSORS AND DONATIONS:
The Chester Summer Basketball League is a non-profit program run by volunteers.  The goal of this league is to provide the basketball playing youth of our area additional playing time in a fun, relaxed  environment where they can continue to develop skills and friendships in the off season.  The league is in need of funds to purchase equipment including game clocks and scoreboards.  Your sponsorship or donation to the league will directly benefit the players.

STATEMENT

I, the parent/guardian of the the child(ren) I register  for The Chester Summer Basketball League hereby give my approval for her/him/them to participate in any and all Chester Summer Basketball activities, including transportation to and from activities.  I know that participation in basketball may result in serious injuries and do hereby waive, release, absolve, indemnify and agree to hold harmless Chester Summer Basketball  and the organizers, sponsors, supervisors, participants, and other persons transporting my child to and from activities for any claim arising out of any injury to my child(ren) whether the result of negligence or for any other cause, except to the extent and in the amount covered by accident or liability insurance.
